Health Assessments

The EAP aims to promote holistic health and well-being by identifying health needs, addressing risk factors, and empowering individuals to take proactive steps towards better health outcomes. Health assessments can aid in evaluating an individual's health status comprehensively. The following evaluations gather information about various aspects of physical, mental, and social well-being:



Depression Screening (PHQ9)

Used for detecting depression and anxiety symptoms in various settings, including general and mental health care.

A brief assessment used to evaluate resilience levels to identify areas of improvement and effective interventions.

Alcohol Use Disorders Identification Test

A screening tool developed by the World Health Organization to assess alcohol consumption, behaviors, and concerns.

Drug Abuse Screening Test (DAST10)

Designed to provide a brief, self-report for population screening, clinical case finding and treatment evaluation.

A screening tool used for determining and hopefully reducing risk of cardiovascular disease for individuals.

A fitness assessment identifies current fitness levels and serves as a baseline to figure out training needs and goals.

A screening tool developed to identify if a person is at low, increased, moderate or high risk of developing diabetes.

Generalized Anxiety Disorder (GAD-7)

The GAD-7 screening is a short assessment used to measure or assess the severity of generalized anxiety disorder (GAD). Individuals are asked to rate the severity of his or her symptoms over the past two weeks.

Screening for Problem Gambling (NCPG)

The National Council on Problem Gambling provides a range of resources, including a gambling behavior self-assessment—a simple ten-question quiz designed to shed light on your gambling habits to understand if it’s time to consider changes or seek support for your behavior.
